Task:

  • Create a list of community resources using MS Word only – for one county (either Macomb, St Clair, Oakland, and/or Wayne) that is readily available for referral to patients and their families.
  • Select any five (05) different community resource types of service listed below.
  • Include a minimum of two (02) examples of each of the five (05) different services with the full name of the Agency/Organization, contact phone number, and website of each community resource
